You may check the cost of your prescription(s) medicine by using our online price list or you can contact our customer service representative. We will require certain information from you which includes:

1) (a) Completed Patient Questionnaire and Personal Information Form
    (b) Signed Canada Medicose Power of Attorney & Release Form

AND

2) Copy of your prescription(s)



You can provide us the above information either Offline OR Online.

(to be downloaded, printed out, filled in and faxed OR mailed to us)
